Foundations: MDP Individual Completion Form

In the following section, you must provide details on how you completed the six courses. If you completed the course at an onsite training, give the course’s area of location. Finally, please provide the completion date if you completed the course online or onsite.

Please be sure to include your pastor and bishop information correctly. When you submit the form, your Regional, National, or State Bishop will receive an email notifying them of your MDP completion. Your pastor will receive an email informing them of your MDP completion and requesting approvalfso that the office of LDD can send you the « Certificate of Achievement. »

When the office of LDD receives the approval from your pastor, you will receive your MDP Certificate of Achievement by email, and your area bishop will receive a copy of your certificate.

Failure to input the correct information will result in you not receiving your « Certificate of Achievement. »
