Please fill this form out if you have completed all six courses of Foundations: Minister’s Development Program. Email the form to ldd@cogop.org. Once the office of LDD receives the form we will then send your “Certificate of Achievement” that recognizes that you have completed the whole program. A copy will also be sent to your state, regional, or national bishop.
After completing this program online, you will be eligible to receive nine credit hours toward a Certificate of Ministry and Theological Studies at Spirit & Life Seminary. After enrolling in Spirit & Life Seminary, complete the Transfer Request Form. Email this form to ldd@cogop.org. After the Transfer Request Form has been approved, the nine credit hours will be transferred to Spirit & Life Seminary.
