Individual Completion Form

Please fill out this form if you have completed all six Foundations: Minister’s Development Program courses. Once the LDD office receives your submission, we will send your “Certificate of Achievement” that recognizes that you have completed the whole program. A copy will also be sent to your state, regional, or national bishop.

Transcript Request Form

After completing the Foundations: Minister’s Development Program online or onsite, you will be eligible to receive nine credit hours toward a Certificate of Ministry and Theological Studies at Spirit & Life Seminary. After enrolling in Spirit & Life Seminary, complete the Transfer Request Form. Once the Transfer Request Form has been approved, the nine credit hours will be transferred to Spirit & Life Seminary.
